Position Description
Job Summary: The Clinical Social Worker provides clinical expertise and support to Community Health Workers working to help low-income, older adults and individuals with disabilities live safely and independently at home. Clinical Social Worker ensures services provided are relevant, effective, accurately documented, integrated and coordinated. Clinical Social Worker is an effective communicator and resource, providing regular consults on program clients requiring intensive case management. Clinical Social Worker partners with Nurse Case Managers in various duties, including conducting home visits, completing comprehensive assessments, building care plans with clients, and supporting ongoing communication between other community care providers. Clinical Social Worker partners with Clinical Care Manager and Director in analyzing program feedback and developing a program model of addressing social determinants of health. Clinical Social Worker uses logic and reasoning to identify strengths and weaknesses while offering alternative solutions to address problems and system gaps.
Essential Functions:
Provide clinical expertise and support to both the client and the Community Health Worker. Ensure services provided are relevant, coordinated, of high quality, meet member needs, are in compliance with RIPIN standards and meet standards/needs of contractors/funders.
Provide support to Community Health Workers and providing services to clients through regular visits to partner agency as well as communications (in-person or by phone) with assigned families when needed.
Cultivate and map resources in Rhode Island that support low-income older adults or disabled adults to the resources needed to live safely and independently at home.
Maintain confidentiality standards and accurate documentation of case records, ensure maintenance of complete and accurate records within agency and program guidelines.
Contribute to the overall success and quality of the agency by working constructively and collaboratively with other staff members ensuring all individuals are treated respectfully and that diversity in backgrounds and life experiences are supported and valued throughout the agency.
Collaborate with program leadership in identifying training needs, particularly in the areas of care coordination and working with complex families, for RIPIN clients and RIPIN partners. Present related workshops as needed.
Actively participate as effective team member in assigned agency meetings and team meetings. Make constructive use of individual and peer supervision/one-on-one meetings.
Accept other duties and responsibilities as assigned.
Knowledge, Skills and Abilities:
A belief in the mission, vision, and values of RIPIN
Advanced knowledge of RIs systems of care, including health, education, special education, employment, and basic needs as they apply across a persons lifecycle
High level of interpersonal skills and excellent communication skills to interact on a professional level with clients, parents and professionals
Ability to convey complex information in a clear and understandable manner
Organizational skills to include managing multiple activities; ability to prioritize projects of varying complexities; ability to meet required timelines
Ability to effectively work independently and in collaboration with multiple staff and programs
Ability to present to small and large audiences that include clients, stakeholders, and professionals on RIPIN programs operations, mission, and focus
Ability to effectively carryout the mission and philosophy of RIPIN and a willingness to work cooperatively and collaboratively with multiple constituents
Proficiency in computer and internet navigation (online documents, search engine, government websites), Proficient in Microsoft products, including Microsoft Office Suite, as well as computer skills to maintain and update program related materials, reports and database
Education and Experience:
Currently licensed by the State of RI as licensed clinical social worker or psychologist preferred; unlicensed applicants must acquire license within one year of hire
Masters Degree or relevant educational experience in related field to carry out job duties and assignments
Experience navigating systems of care in RI supporting low-income older adults or disabled adult to live safely and independently at home
Experience in behavioral health settings desired
Personal experience navigating social, healthcare, and/or education systems and services for self or family member highly desired
Demonstrated knowledge of RIPINs programs, mission and vision
Experience working with and outreaching to diverse populations
Fluency (speaking, reading, and writing) in English and Spanish required
Physical Demands:
While performing the duties of this job, the employee is regularly required to talk or hear. The employee regularly is required to stand, walk, sit, use hand to finger, handle or feel objects, tools, or controls; and reach with hands and arms. While performing the duties of this job, the employee is regularly required to climb stairs, reach, stretch, stand and bend. The employee frequently lifts and/or moves up to 25 pounds. Clinical Social Workers are required to conduct home and community visits and travel between multiple provider sites. Must be able to provide reliable transportation to facilitate visits to clients home or community setting and climb up to three flights of stairs.

Working Conditions/ Work Environment:
Primary work location is a climate-controlled indoor office environment; however, may also be required to conduct visits in private homes, climbing up to three flights of stairs, and community locations
Flexibility for occasional travel related to job requirements
Willingness and ability to work evenings and weekends as needed
Provide own reliable transportation with proof of RI minimum requirements of auto insurance
